Two-Week Self-Assessment Status – Confirmation

Thank you for following up for your two-week self-assessment . Your form has been submitted and will be reviewed by one of our nurses. We will contact you if you requested for us to do so.

Remember:  In two more weeks you should take your UPT (urine pregnancy test).  If the test shows positive, please call us to inform us and make an appointment.
