Requesting your appointment:

  1. Fill in the basic requested information below.
  2. Select Gynecological Care or Abortion Care appointment. You will see options to request appointments.
  3. All appointment requests are reviewed by a Patient Educator.
  4. You will be contacted (via your preferred method) to confirm your appointment time.
